Please print and complete the following form(s) if your child suffers with Asthma or an Allergy, the forms can be found below:

Medical Consent Form

If you would like for your child to have the option to take medicine during school hours, please complete a Medical Consent Form (below). This form is necessary to ensure that we have your written consent for your child to administer medication for themselves supervised by a staff member during school hours if the need arises.
